After You Enroll: Complete Your Health Review

Complete your secure health questionnaire so your clinician can review your medical history and determine whether treatment is appropriate.

Enrollment does not guarantee a prescription. Your clinician must review your health information before medication is prescribed or treatment begins.

Cancellation Policy

Tablet and in-office memberships automatically renew until canceled: monthly for tablet programs and every 28 days for in-office injection programs.
Cancellation requests must be received at least 5 days before your next renewal date to prevent the next scheduled charge.
Once a membership has renewed or medication has been ordered, prepared, dispensed, or allocated for treatment, payment is nonrefundable.
For in-office injection memberships, unused or missed weekly injection visits do not roll over to future membership cycles and are not eligible for refunds or credits.
Cancellation stops future renewals but does not refund the current membership cycle.
Treatment may be modified or discontinued if the provider determines that continued treatment is no longer medically appropriate.
