For Healthcare Providers

Working Together for Better Patient Care

At Lakeland Birth & Gynecology, I believe good patient care often starts with good communication between providers.

Whether you're looking to refer a patient, coordinate treatment, discuss prescription-related needs, or explore opportunities to collaborate, I'd love to connect.

My goal is to make working together straightforward while helping patients receive thoughtful, individualized care.

Are You a Local Provider With a Limited Formulary? Let's Talk.

Do you care for patients but occasionally encounter limitations with your prescribing formulary or available treatment options?

I'd love to connect and explore how we may be able to work together.

Whether you're looking for an appropriate referral option, additional treatment resources, or a collaborative approach to patient care, I'm always happy to connect with other healthcare professionals in our community.

My goal is to support both patients and their providers through communication, collaboration, and evidence-based care.

All prescribing and treatment decisions are made following an appropriate clinical evaluation and in accordance with applicable prescribing laws and professional scope-of-practice requirements. Collaboration does not bypass another provider's legal or clinical restrictions.

Referrals and clinical collaboration

Connect about women's health, gynecology, primary care, prenatal or postpartum care, iron infusions, existing orders, and prescription-related consultation. Every request receives independent clinical review.

Prescribing requires an appropriate patient-provider relationship, clinical evaluation and documentation. A request does not guarantee prescribing, dispensing or treatment.

A careful approach to documentation

  1. Send your professional contact information and a general description using this on-site form.
  2. We contact your office to verify the request and appropriate authority.
  3. We coordinate an approved secure channel for any necessary clinical records before review or scheduling.

Provider document submission has not been verified. Do not send prescriptions, signed orders, patient records or labs through this form or ordinary email. Existing patients may use the Patient Portal; provider access or authorization must be arranged separately.

For coordination without patient details, call (863) 666-7555. Urgent clinical needs require the appropriate direct clinical or emergency channel, not this form.
