After You Enroll: Complete Your Health Review

Complete your secure health questionnaire so your clinician can review your medical history and determine whether treatment is appropriate.

Enrollment does not guarantee a prescription. Your clinician must review your health information before medication is prescribed or treatment begins.

Cancellation Policy

  • Tablet and in-office memberships automatically renew until canceled: monthly for tablet programs and every 28 days for in-office injection programs.
  • Cancellation requests must be received at least 5 days before your next renewal date to prevent the next scheduled charge.
  • Once a membership has renewed or medication has been ordered, prepared, dispensed, or allocated for treatment, payment is nonrefundable.
  • For in-office injection memberships, unused or missed weekly injection visits do not roll over to future membership cycles and are not eligible for refunds or credits.
  • Cancellation stops future renewals but does not refund the current membership cycle.
  • Treatment may be modified or discontinued if the provider determines that continued treatment is no longer medically appropriate.
